Your First Appointment
In order to make your first visit as pleasurable and efficient as possible, we request that you arrive at least 20 minutes prior to your initial visit so that you may find parking and be properly registered. In addition, please read the attached information and complete the patient forms and questionnaire.
Office Hours and Parking
Our regular office hours are Monday - Thursday, 8:30 a.m. - 4:30 p.m. and Friday
8:30 a.m. – 1:00 p.m. There is a parking garage under the building; however, we do not validate the parking tickets. The average parking cost per visit is $4.00 (cash or check).

Who Pays For Your Care?
Our practice accepts payment from a number of medical insurance plans. If we are a participating provider on your plan, we will accept your valid insurance card as payment for the services we provide to you. Patients will be responsible for any co-payments, out of network fees, deductibles, additional testing and services not covered by your insurance company.
If you do not have medical insurance or if you have forgotten to bring your insurance card, you will be considered a Self-Pay patient. Self-pay patients must pay for the medical services they receive from us at the time of the visit. We accept cash, personal check, VISA, Discover, Mastercard, or Care Credit.

We understand that nobody enjoys the hassle of filling out paperwork. However, for your convenience we have provided a link to our forms so you can print and complete the forms before your visit. Please remember to bring the completed forms, insurance card(s), and current medications with you on your first visit. Please print the following forms:
Insurance and Payment Information
We accept all insurance products, with the exception of all HMO's and Medicaid. Please click here to see a list of our IN NETWORK insurance carriers that we accept. However, most plans that are not HMOs have a benefit design that would reimburse for some of Dr. Boone’s services. If your insurance is not listed or if you are unsure of the type of insurance you have, please call our office during business hours for more information.
