Your First Appointment
In order to make your first visit as pleasurable and efficient as possible, we request that you arrive at least 20 minutes prior to your initial visit so that you may find parking and be properly registered. In addition, please read the attached information and complete the patient forms and questionnaire.

Office Hours and Parking
Our regular office hours are Monday - Thursday, 8:30 a.m. - 4:30 p.m. and Friday
8:30 a.m. – 1:00 p.m. There is a parking garage under the building; however, we do not validate the parking tickets. The average parking cost per visit is $4.00 (cash or check).

Who Pays For Your Care?
Our practice accepts payment from a number of medical insurance plans. If we are a participating provider on your plan, we will accept your valid insurance card as payment for the services we provide to you. Patients will be responsible for any co-payments, out of network fees, deductibles, additional testing and services not covered by your insurance company.

If you do not have medical insurance or if you have forgotten to bring your insurance card, you will be considered a Self-Pay patient. Self-pay patients must pay for the medical services they receive from us at the time of the visit. We accept cash, personal check, VISA, Discover, Mastercard, or Care Credit.

Insurance and Payment Information
We accept all insurance products, with the exception of all HMO's and Medicaid. Please click here to see a list of our IN NETWORK insurance carriers that we accept. However, most plans that are not HMOs have a benefit design that would reimburse for some of Dr. Boone’s services. If your insurance is not listed or if you are unsure of the type of insurance you have, please call our office during business hours for more information.

Our goal is to keep your insurance or other financial arrangements as simple as possible. In order to accomplish this in a cost-effective manner, we ask that you adhere to the following guidelines:

1. You are ultimately responsible for payment of charges for services you receive from our office. Certain procedures are non-covered services under all insurance policies; therefore, payment is expected at the time of service.
2. If a service is a non-covered service, there is no diagnosis code that will cause your insurance company to pay.
3. It is your responsibility to provide us with your current address, telephone number, social security number and insurance information at each visit.
4. It is your responsibility to contact your insurance carrier to confirm that the doctor you are seeing is a participant of your plan. If the physician is not currently on your plan, you will be responsible for payment in full.
5. We will mail you a monthly statement for any outstanding balances. If your insurance carrier has not paid the claim within 30 days of the date of service, please contact your carrier and assist us in getting the claim paid.
6. We will only file insurance up to 3 months per claim. At that time, payment is due in full by the patient.
